Creating and Completing Assessment Forms
The ability to create Assessment Template is dependent on the role of the user and the permissions granted to that user.
Creating a New Template
If you click on Templates you will see all of your existing Assessment Templates plus a ‘New template’ in the top right hand corner of the screen.
To create a new template click New template.
Assessment Form Templates
- Where you see the
icon, you are able to click and insert text
- Name: Assessment name
- Description: A description of the Assessment
- Body: The questions you wish to appear on your Assessment form
- Summary: Will display upon completion of the Assessment forms
- In the main body, you may amend the name of each tab by clicking on the
- To create additional tabs, click on ‘New +’
- To add questions to your tabs, drag across the desired toolbox type from the left hand menu e.g. Dropdown.

Checkbox |
Allows creation of a pre-defined item with a tick box next to it. In Phase 1, where more than 1 answer maybe applicable (e.g. allergies) then more than one checkbox maybe dragged to the same area and labelled thus allowing more than 1 selected answer in a section or question |
Numberpicker |
Allows input of numbers only – if something else is input whilst processing an Assessment, you will be prevented doing so |
Datepicker |
Allows selection of a date with date picker popping up upon clicking on date field |
Dropdown |
Allows the selection of the single most appropriate answer by clicking on it. This type allows scoring to be set against it to set up recommended follow up instruction in Summary |
Combobox |
Works in the same way as a dropdown list but also allows free text input of your own choice by typing it in, if the original list is insufficient |
Radiobuttons |
Allows selection of 1 item from a pre-defined list by clicking radio button. This type allows scoring to be set against it to set up recommended follow up instruction in Summary |
Completing Assessments
To create a Service User Assessment:
- Navigate to Service Users and select a Service User.
- The Service User list will condense to the left and the Service User details will populate the right hand side of the screen.
- Scroll down to Start New where a list of all available Assessment Templates will display.
- Below Start New you will see an area headed Assessments - this is where Completed or In Progress Assessments are listed.
- There is an ‘Available offline’ button that allows access to that Service User Assessments at times when your Mobile Device is offline, to enable this click Available offline’
- Single click on the Assessment that you wish to process - this will open the Assessment for you to populate.
