Edit Assessment Form Page Template
Care & Clinical allows you to refine our assessment form templates to better meet your organisational needs. If you do not require one or more of our assessment form templates, you can archive them and if one or more of your assessment forms are missing, you can build them.
The user can select an Assessment Form name from the Create/Edit Assessment Form page to edit an Assessment Form.
- Click Admin
- Browse to Clinical Configuration
- Click Create/Edit Form
- Underneath the "Questions" column, click Edit
From the Edit Assessment Form Questions page you are able to:
Add a Question:
To Add a new question click Add a question, at the top or bottom of the page.
When selected, you will be taken to the bottom of the page and a blank question will be displayed and maximised. The question will be numbered accordingly. When you Save the new Question, a new version of the Assessment Form will be created and any member of the team that completes the Assessment Form will see the newly added question.
Delete A Question:
To Delete a question, expand the question that you wish to delete and click Delete Question. A prompt will display to confirm that the delete is to occur. When you Save the new Question, a new version of the Assessment Form will be created and any member of the team that completes the Assessment Form will see the question has been removed. Deleting a question from a form will automatically strike-through the associated responses in all residents’ active Care Plans.
Move Responses:
When the response type for a question is drop down or multi-select, the responses can be entered and then moved. Click the grey three lines icon to drag and drop each response into your preferred order.
Edit Questions:
When editing a question, a prompt will display asking if the edit has changed the meaning of the question. If punctuation, grammar or spelling has been changed and it is obvious that the meaning of the question has not changed, selecting ‘No’ and saving the change will not update the Assessment Form version. If the meaning of the question has changed then selecting ‘yes’ and saving will update the Assessment Form version. This prompt will also display when editing a multi select or drop down response.
Add Scores:
All drop-down and multi-select responses include the option to add a score. If you wish to add a score, type the score into the score field next to the response. Once saved, the Assessment Form will display the score in brackets next to the response. As you complete an Assessment Form and select responses associated with scores, the total score will display at the bottom of the Assessment Form.
Note: If you make any changes to the Form Questions, please remember to click Save before leaving the page.
