Create/Edit Care Domains List Page
Care Domains are used to break Care Plans into sections. As part of your Assessment Form design process you can map assessment responses to different areas of specific Care Domains in order to dynamically build and maintain each of your resident's care plans.
- Move Order of Care Domains and Impact to Active Care Plan
- Create a New Care Domain
- Add, Edit and Move Library Goals + Interventions
- Add a New Sub Domain
- Archive a Care Domain
Move Order of Care Domains and Impact to Active Care Plan
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Care Domains
- Using the grey three lines icon
to drag and drop each Care Domain into your preferred order
- The evaluate, extended and summary views will display the new order of the Care Domains.
- The unfinished version of the Care Plan will display the new order of the Care Domains.
- Any historical Care Plan view will NOT be updated and will retain the order of the Care Domains as at the day the Care Plan was made historical.
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Care Domains
- Click Add a Care Domain
- Enter a Care Domain Name
- Click Save
Note: Once the new Care Domain name has been entered and save has been selected, the Care Domain will be created. A Care Domain with the same name as a current active Care Domain cannot be created. Once the Care Domain is created, it will be available to select in Assessment Form mapping and within the Care Plan builder pages.
Add, Edit And Move Library Goals + Interventions
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Care Domains
- Click the name of the appropriate Care Domain
- To add a new library Goal, click Add New next to "Goal"
- To add a new library Intervention, click Add New next to "Intervention"
- To update the order of both library Goals and Interventions, use the grey three lines icon
to drag and drop each Goal or Intervention into your preferred order
Note: Once a Goal or Intervention has been added each can be edited by selecting the relevant edit link. Once selected the text will be edited and select the save button to save. To remove a goal or an intervention, select the remove link. You will be prompted and will need to select the remove button. Once selected the Goal or Intervention will be removed. As it is a Care Domain the Goal or Intervention is not struck through.
If you remove a Goal or Intervention that exists in an active Care Plan it will not be automatically deleted from the active or unfinished Care Plan. Instead you will be required to manually remove this data from the Care Plan.
Sub domains enable organisations to reduce the amount of Care Domains and enable grouping of Goals, Interventions and Observations within a resident Care Plan.
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Care Domains
- Click the name of the appropriate Care Domain
- Click Add New Sub-Domain
- Enter a Sub-Domain Name
- Select Bold if you would like the Sub-Domain name to appear in Bold
- Select Red if you would like the Sub-Domain name to appear in Red
- Click Save
Note:
- There is no restriction on the number of Sub-Domains per Care Domain
- Each Sub-Domain heading MUST be unique. If a Sub-Domain name is entered that matches one already entered within the Care Domain, it will not be able to be saved.
- Once the Sub-Domain heading has been saved, it will be available within the Assessment Form template for mapping
- Once the Sub-Domain heading has been saved, it will be able to be viewed and added into the Care Plan through the Care Plan builder
- Once the Sub-Domain heading has been saved, it will be able to be viewed within the active Care Plan evaluate view. It will only display within the active Care Plan extended and summary view if there is manually entered or Assessment Form data within it
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Care Domains
- Place a check in the appropriate box in the Archive column
- The "Archive" button will become active
- Click Archive
Note: You can view all Archived Care Domains, by changing the status on the Care Domains page from "Active" to "Archived".
To Restore a Care Domain, follow the same process as above on the "Archive" page, but instead of clicking Archive, click Restore.
If the Care Domain currently has Assessment Form mappings, the archive checkbox will be replaced with the "Un-map" link. Prior to archiving a Care Domain all Assessment Form mapping must be removed. When selecting the "Un-map" link, the Care Domain form impacts page will display.
This page will display:
- Each Assessment Form that has mappings to this Care Domain
- Each question within the Assessment Form that is mapped to the Care Domain
- Whether that question is mapped to the Intervention, Goal or Observation
- There are several different options for remapping the questions:
- All items mapped to the Care Domain – this will select every question within every form
- All items mapped to a particular Assessment Form only
- Individual questions
When the selection has been made, you can select to re-map the selected items to a new Care Domain and/or Sub-Domain or delete the mappings all together. To remap the selections,
- Click Un-Map
- Select the new Care Domain to map the questions to using the Re-map selected items to: drop down list
- Select a Sub-Domain if appropriate
- Place a tick in the appropriate Selection checkboxes
- Click Update
- Once all items linked to the Care Domain have been re-mapped, the Care Domain can be archived.
