Dynamic Care Plans
Dynamic Care Plan is a feature that is enabled per Assessment Form. If enabled, each time you complete an Assessment Form, the responses that are captured will map automatically to the Resident's Active Care Plan, thus ensuring the Care Plan is always showing the most recent information relevant to the needs of the Resident.
Each time new information enters the Care Plan, the previous information will be struck through automatically and maintained for audit purposes. The stuck through data can be seen through the Evaluate Care Plan view.
Enabling Dynamic Care Planning
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Form
- Select the Assessment Form that you would like to make Dyanmic
- Change the Update Care Plan drop down as per the below options
- Click Save
Do Not Update Care Plan - when the Assessment Form is completed the information will not automatically update the Care Plan and instead must be added manually
Update Active Care Plan - when the Assessment Form is completed any responses that have been mapped to a Care Domain or Sub-Domain will automatically update the Care Plan
Prompt to Update Active Care Plan - when the Assessment Form is completed, you will be asked whether you would like the information captured to automatically update the Care Plan. If you select yes, any responses that have been mapped to a Care Domain or Sub-Domain will automatically update the Care Plan
Mapping Assessment Form Questions/Responses To The Care Plan
- Click Admin
- Browse to Clinical Configuration
- Click Create / Edit Form
- Click Edit to the right of the appropriate Assessment Form
- Click the expand (+) icon
to the right of the question you want to link to a Care Domain or Sub-Domain
- Browse to Mapping
- Use the drop down to select the Care Domain or Sub-Domain that you wish to map the response to
- Click Save to save the Assessment Form template
Viewing Struck Through Information
- Click Residents
- Browse to the appropriate Resident
- Click Care Plan
- Click Evaluate
- Click the expand (+) icon
to the right of the Care Domain you wish to revise strike through information
- Put a Check in the Show care domain strike throughs checkbox
Notes:
- Each time the Assessment Form is completed the mapped responses will update the active Care Plan immediately and the previous data that has been updated will be struck through.
- If the you map the Assessment Form response to a different Care Domain, the data within the active Care Plan views will immediately be changed. Any Assessment Form data will now display within the new Care Domain.
- If a new sub domain is added to a Care Domain and Assessment Form responses are mapped to the new sub domain, all Assessment Form data within the active Care Plan views will be updated to reflect that change.
- If a grammatical or spelling error is fixed within an Assessment Form question via the template this change will immediately be reflected within the Care Plan, if the Assessment Form version has not changed.
- If a completed Assessment Form is archived, any mapped data will remain within the active Care Plan.
- If an Assessment Form Template is archived, any mapped Assessment Form data will be struck through with the date and time and username of the person who archived the Assessment Form Template.
- If an Assessment Form is completed with data mapped to a Care Domain that is currently not displayed within the active Care Plan, the Care Domain will now automatically display together with the mapped Assessment Form data.
- Users with permission will be able to update an Assessment Form directly from the Care Plan. When completed, all the Assessment Form data will be updated within the Care Plan with the new date and user details.
- Historical Care Plans will not be updated at any time, all manually entered data, Assessment Form data, Evaluations and Strike Throughs will remain as at the time and date the Care Plan become historical.
- If a Care Domain is archived and there is manually entered data within that Care Domain, the Evaluate view will display with a message stating that the Care Domain has been archived. This gives the user the opportunity to move or delete the manual data. If the data remains within the archived Care Domain, it will continue to display on the Care Plan for that resident.
