POST-MORTEM:
Between 15:00 pm and 18:07 pm on 9 April 2019, a service issue impacted users being able to login to Mobile Point of Care. The issue did not impact users already logged into Mobile Point of Care.
The issue was caused by a routine server upgrade taking longer than expected. This type of upgrade is one that our team have completed numerous times before without issue. After some investigation, a decision was made to continue with the upgrade as opposed to rolling it back – this decision was made because continuing was the quicker option.
To mitigate against a recurrence of this problem, we will be making some changes to server upgrades in the future:
1. Server upgrades will be completed during periods where there is little to no user activity (later at night / early morning).
2. Additional resource will be added to services during upgrades to speed up the update process.
