Using Actions To Complete Reassessment
Assessment Forms can be configured to use Review Dates so that the team are prompted [via Actions] when an Assessment Form is due for Reassessment.
In starting the action, the team are automatically taken to the Assessment Form prefilled with the last set of data that was captured to make reassessing and updating appropriate questions a seamless and efficient process.
Note: Reviewing or Editing an Assessment Form will not delete previously captured information. All previously captured information is retained and can be accessed from the Assessment Form tab in the Resident's record or via the Documentation menu.
- Navigate to the Current Actions screen
- Click on an Assessment From Action
- Click Start This Action
- Once the Assessment Form loads, update all responses as appropriate
- Enter a new Review Date
- Enter your Password
- Click Complete
- The Assessment Form will be saved and a new Action will be completed for the Review Date specified above
Note: If you are using Dynamics Care Planning, completing the Reassessment will automatically update the Care Plan as per the changes you make. If you make no changes, the Care Plan will remain unchanged.
